Complete all of the online forms – click the “RANK ONE” button to be directed to the instructions for online forms on Rank One.

Next, click the “Fulton County Forms” button for the Pre-participation Physical Form. All of these forms are required by Fulton County Schools. All forms must be completed and approved by the Milton trainers prior to the first day of practice, October 20th.
